ABSTRACT
OSIA,WILLIAM M. “ SERVICES OF THE COST CENTERS AT THE BICOL REGIONAL TRAINING AND TEACHING HOSPITAL (BRTTH)’ ( Unpublished Master Thesis, Aquinas University, Legaspi City, March 2002)
The study is centered on the services of the cost centers of the Bicol Regional Training Hospital 9 BRTTH) in Legazpi City. It aimed to know whether the priority programs and activities are efficiency implemented and the organizational and cost effectiveness are prioritized.
As a proof that the organization is decentralized and autonomy is practiced at the division or section levels, the cost centers have been formed mainly to generate information on product coast and income.to facilitate the planning and control, the assignment of cost was done, with the influence of planning and control, the assignment of cost was done, with the influence of the centers and decision-making. The decision making is made on what product to develop, what production technology to use, what product mix to sell and what price to charge.
The main objective of the is to look into the operation of three selected cost centers of the hospital. Specially, it answered the following problems:
1. What are the services offered by the cost centers at the Bicol regional Training and Teaching Hospital?
2. What is the average cost per patient served at the cost centers per day for the:
a. In-patients; and
b. Out-patients?
3. What are the problems encountered by the patients and personnel servicing the cost centers?
4. What are the possible solutions that would address these problems?
The study was focused on the three cost centers only namely- the pharmacy, laboratory and the radiology that service the in-and out-patients. The timeframe was limited to June and July of the year 2001. It used documentary analysis and descriptive survey method. The respondents were the out-patients. The respondents were the out-and in-patients as well as the personnel assigned at the three cost centers.
Findings
The findings of the study were:
1. The cost centers at the BRRTH offered prescription and dispensing services at the pharmacy. Screening and examining of specimen at the laboratory and X-ray and ultrasound services at the radiology center both for in and put patients of the hospital.
2. The average cost services per patient per day for in patient is relatively high as compared to out-patient particularly at the pharmacy department followed by radiology services and least in laboratory services.
3. Both in-and out patients and personal at the cost centers encountered problems in the discharge of their functions, to wit;
3.1 Inadequate supplies and equipment;
3.2 Manpower’s shortage to apply provide for equality patients care;
3.3 High Cost of medicine and services;
3.4 Long queue of patients to be served;
3.5 Poor human relation and lack of discipline; and
3.6 Inadequate funds.
4. Both in-and out-patients and hospital personnel at the cost centers recommended solutions to problems encountered , to wit:
4.1 Hire additional manpower, casual and contractual employees, if not permanent;
4.2 Procure adequate supplies including medicines and equipment;
4.3 reduce cost medicine/services;
4.4 have continuing staff development;
4.5 pay overtime services; and
4.6 Implement fully the Magana Carta for Health Workers.
Conclusions
1. The pharmacy, laboratory and radiology services offered by the cost centers of the BRTTH that were well availed by in and out patients are found inadequate as compared to the volume of patients served.
2. The relatively high cost of pharmacy services followed by radiology and laboratory services or in patients is due to added cost of medicine, laboratory examination and radiology examination as maybe warranted for confinement.
3. Problems in the discharge of their functions always best government hospital like other government agencies. No agency for that matter can boast of an absence of a problem in the organization as it grows with challenges and change.
4. The problems beset by the in patients and out patients as well as personnel who manned the cost centers have solutions. They can either be solved by the agency or with the support of higher authorities.
Recommendations
1. The pharmacy, laboratory and radiology services need to be enhanced through the presence of adequate supplies, equipment and manpower.
2. The BRTTH being a tertiary hospital and a government hospital, whose vision is “to serve as a model of humane hospital with particular bias for the poor” should adopt and implement the socialized billing scheme for the client to address the identified problems on high cost of medicine as well as services.
3. The BTTH should find ways to address problems that can be resolved by and within the agency itself. Those that need policy reformation, decision, making maybe forwarded to appropriate authority. A continuing dialogue, staff development training and seminars would greatly enhance the services of the hospitals and
4. The BRTTH should exert to resolve problems identified by tapping its immune resources, particularly the manpower services and the cost centers as its good sources of additional funds, if the socialized billing is implemented.
Areas for further study:
Based on the foregoing findings, the following areas for further study maybe considered:
1. Institution of the Socialized Billing System among government hospital;
2. Information Technology Resources for the BRTTH;
3. A replication of these study for other cost centers at the BRTTH;
4. A replication of this study in other government hospital at different level; and
5. A research on client satisfaction on BRTH services.
